I too had a hard time following the firmware installation.

That looks correct. I would almost suspect a config.g issue. Does yours look like this? ; Axis and motor configuration M569 P0 S0 ; Drive 0 goes forwards (X tower) M569 P1 S0 ; Drive 1 goes forwards (Y tower) M569 P2 S0 ; Drive 2 goes forwards (Z tower) M569 P3 S1 ; Drive 3 goes forwards (extruder 1)

Just out of curiosity, take a picture of one of your carriages and post it. May help narrow down the problem.

I think I figured it out on my own. My Duet Ethernet came with an SD card for the WIFI version of the Duet. Between the RepRap and Duet WIKI's I was able to get it up and running. Instructions specific to the Ethernet version of the Duet board would have saved some time and frustration. Correct file...

I just completed building my UMBCs and only needed to purchase the afore mentioned eccentric nuts. The 6mm eccentrics do need 3mm spacers, but I just used the extra washers from the open builds wheel kits that came with the D300VS kit.
